Guide To Ordering Additional Services

In addition to Donor Sperm, Manhattan CryoBank offers extensive services pertaining to Cryopreservation, Andrology, and Specimen Transportation. Our Cryopreservation Services, also referred to as Sperm Storage and Egg/Embryo Storage, perserve sperm, eggs, and embryos for future use. Our Andrology Services, which is the medical specialty that focuses on the male reproductive system, help our male clients with reproductive and urological problems specific to men. Our Specimen Transportation Services, also referred to as Shipping Services, transport specimens to and from Manhattan CryoBank, as well as between clinics.    

Below is an overview of the neccessary steps to take if you are seeking Cryopreservation Services, Andrology Services, or Specimen Transportation Services.

Cryopreservation and Andrology Services

If you are interested in completing Cryopreservation (for sperm, eggs, or embryos) and/or Andrology Services with Manhattan CryoBank, please see below for a simple guide explaining the necessary steps.

Step 1: For Cryopreservation and/or Andrology Services, you must first obtain a referral/prescription from your doctor that states which type of service is to be performed. The doctor’s referral/prescription can be faxed, emailed, or brought with you to Manhattan CryoBank on your first visit.

Step 2: After you have submitted your doctor’s referral prescription, you must contact Manhattan CryoBank to schedule your appointment. For Sperm Storage and Andrology Services, we recommend 2-5 of abstinence prior to appointments.

Step 3: When you arrive at Manhattan CryoBank, you will be provided with the appropriate documents to complete. Please make sure to have your photo ID with you when you visit our facility.

Sperm, Egg, and Embryo Transportation

Manhattan CryoBank can assist you with transporting your specimens to and from Manhattan CryoBank, as well as between clinics. You can find transportation prices listed on our Additional Services Fees page. 

You may ship specimens that you have in storage at Manhattan CryoBank through our Order Additional Services page.

If you will be transporting specimens from another facility to be stored at Manhattan CryoBank, you will need to follow the steps below:

Step 1: Please contact Manhattan CryoBank for a quote and to discuss the transport. During this discussion, we will assess if your transport will require an exemption from the NYS Dept. of Health. Given that we are located in New York State, the NYS Dept. of Health requires all specimens to be produced by a NYS licensed clinic or doctor. If your existing clinic is within New York State, we typically do not need an exemption. However, if your clinic is located outside of New York State, we will need to determine if an exemption needs to be submitted. If an exemption is needed, the cost for submission is $100.

Step 2: If you decide to go ahead with the transfer, we recommend speaking with your fertility clinic or fertility doctor’s office to make sure you have completed all of the necessary paperwork to release the specimens.
